Founded in 1994, LifeWise Health Plan of Oregon is a subsidiary of Premera Blue Cross. The company currently serves 160,000 members across all counties of Oregon. Between LifeWise and its affiliates, the group serves 1.5 million members across its network. The LifeWise Health Plan of Oregon network covers Oregon, Washington, Alaska, and Arizona.

LifeWise Health Plan of Oregon offers a variety of individual plans including Preferred Provider Organization (PPO) plans, Traditional Indemnity plans, and Supplemental Benefit Options. LifeWise PPO plans include the Plus Plan, Preferred Plan, Prime Plan, Value Plan, and Health Savings Account (HSA) Plan. Traditional Indemnity plans include The Choice and HSA Choice.

LifeWise Plans

LifeWise PPO plans feature access to a large network of health care providers, hospitals and physicians, flexible designs, and coverage based on percentages of expenses after annual deductibles.

  • PPO Plus - This plan features five deductible choices, ranging from $500 to $7,500. Preferred co-payments and percentage of member responsibility for this plan includes a $20 co-payment for preventive care, 20 percent of hospital services, hospital emergency, maternity, and $20 copay for urgent care. In addition, PPO Plus has no balance billing and easy claim filing. No referrals are required, and members, when traveling, have access to coverage.

  • PPO Prime - This plan offers three deductible choices, $2,500, $5,000, and $7,500. Member responsibility percentages are slightly higher at 30 percent, with similar benefits as the PPO Plus.

  • Traditional Indemnity - Considered the most cost-effective LifeWise option, these plans feature a variety of coinsurance, benefits, and deductibles. No referrals are required and members have the freedom to choose their own providers and a flexible benefit design. A separate $500 deductible applies to prescription services.

  • HSA - These high-deductible plans are designed to keep premiums low. HSAs are considered a cost-effective method for providing members with preferred provider and non-preferred provider coverage for major illness. Other HSA plan benefits include: tax savings, retirement savings, reduced insurance premiums, and better choices regarding treatment options.

There are several supplemental benefit options worth mentioning for individuals interested in an additional prescription drug plan or for those seeking treatment for alcohol dependency.

  • Prescription Drug Buy-Up - This option allows members to purchase prescription drugs without a yearly minimum or deductible. While unavailable with HSA plans, it may be added to existing member plans for an extra cost.

  • Alcohol Dependency Treatment Plan - Offered in the state of Oregon, this treatment plan is an optional benefit featuring coverage of up to $4,500 during any 24 consecutive months.
