Midwest Security Plans

Midwest Security offers a myriad of plans throughout its coverage area:

  • Select Coverage 80%/60% - A PPO for Nebraska residents. Deductibles range from $250 to $2,500. For in-network treatment and office visits including urgent and routine care, coverage is typically 100 percent after the copayment. There is, however, a $500 maximum for routine examinations. For out-of-network, most services are covered 60 percent after the deductible has been reached. Hospital services are covered 80 percent in network and 60 percent out-of-network after deductibles are met. For medication, copayments can range anywhere from $10 to $50.

  • Select Coverage 2000 90%/70% - A PPO for Iowa and Northwest Illinois residents. Deductibles range from $250 to $5,000. After the deductible has been met, this plan covers 90 percent of in-network costs and 70 percent of out-of-network costs. Medications are covered, after a $10 to $50 copay.

  • Classic One 90%/70% - A PPO for Wisconsin and Ohio residents. Deductibles range from $250 to $5,000. Office visits are covered 90 percent after the deductible for in-network care, and 70 percent after the deductible for out-of-network care. There is a $50 copay for in-network emergency care and $75 for out-of-network emergency care. Medications are covered with a $10 to $50 copayment. For drugs with a generic equivalent, members who choose the brand name drug must pay the difference in cost between the generic and brand name versions.
